XDOT Academy WAIVER AGREEMENT:

I, the undersigned, being over the age of 18 years, acknowledge the inherent risks associated with participating in the activities offered by XDOT ACADEMY, including but not limited to Co-op, Sports Teams, Sports Leagues, Sports Camps, Teen Nights, Kids Events, Classes, Clubs, and any other activities provided by XDOT ACADEMY.

In consideration of XDOT ACADEMY permitting my child to participate, I voluntarily assume all risks associated with any and all programs, activities, and events organized by, associated with, or on behalf of XDOT ACADEMY, taking full responsibility for any injury or accident that may occur to me or my child(ren) during our participation. I knowingly and intentionally release and waive any and all claims, of any kind or nature, against XDOT ACADEMY, its Incorporators, Board of Directors, employees, volunteers, agents, and representatives, arising from or related to the XDOT ACADEMY Program. This release and waiver extend to my heirs, administrators, and assigns.

I further indemnify, hold harmless, release, and discharge XDOT ACADEMY, its Incorporators, Board of Directors, volunteers, agents, employees, and officers from any and all claims, demands, liabilities, actions, judgments, expenses (including attorneys’ fees and defense costs), and executions arising directly or indirectly from my child’s participation in, or association with, the XDOT ACADEMY Program, regardless of the act, neglect, default, or omission of any person, firm, or corporation associated with XDOT ACADEMY.

PHOTO RELEASE

By signing below, I grant permission to XDOT ACADEMY to use photographs and/or video of me or my child taken at XDOT ACADEMY Programs in publications, news releases, online, and other communications related to the mission of XDOT ACADEMY.

I HAVE READ AND UNDERSTAND THIS AGREEMENT, AND I AM AWARE THAT BY AGREEING TO THIS FORM AND PARTICIPATING IN XDOT ACADEMY AS DESCRIBED HEREIN, I AM WAIVING CERTAIN SUBSTANTIAL LEGAL RIGHTS THAT I, AND MY HEIRS, PERSONAL REPRESENTATIVES, AND ASSIGNS, MAY HAVE AGAINST XDOT ACADEMY.
