XDOT ACADEMY ATHLETICS

WAIVER OF LIABILITY AND RELEASE AGREEMENT

The undersigned, being over the age of 18 years, hereby acknowledges that there are certain risks in participating in XDOT ACADEMY Athletics. In consideration of XDOT ACADEMY allowing my child to participate in XDOT ACADEMY Athletics, I hereby assume all risks associated with XDOT ACADEMY Athletics, including but not limited to, full and complete responsibility for any injury or accident which may occur to my child in connection with XDOT ACADEMY Athletics. I knowingly and intentionally hereby release and waive any and all claims, of whatsoever kind or nature that I may have against XDOT ACADEMY, its Incorporators, Board of Directors, employees, volunteers, agents and representatives, resulting in whole or in part, from participation in XDOT ACADEMY Athletics. This release and waiver shall also be binding on my heirs, administrators, and assigns. In this regard, I indemnify, hold harmless, release and discharge XDOT ACADEMY, its Incorporators, Board of Directors, volunteers, agents, employees, and officers, from any and all claims, demands, liabilities, actions, judgments, expenses (including attorneys’ fees and costs of defense), and executions which may be made by reason of any injury to my child (including, but not limited to, serious bodily injury or death), or damage to property sustained by my child, caused by any act, neglect, default, or omission of any person, firm, or corporation, directly or indirectly associated with XDOT ACADEMY, arising directly or indirectly out of participation in, or association with XDOT ACADEMY Athletics.

PHOTO RELEASE

By signing below I hereby grant permission to XDOT ACADEMY to use photographs and/or video of me or my child taken at XDOT ACADEMY Athletic events in publications, news releases, online, and in other communications related to the mission of XDOT ACADEMY. 

I HAVE READ AND UNDERSTAND THIS AGREEMENT AND I AM AWARE THAT BY AGREEING TO THIS LIABILITY RELEASE AGREEMENT I AM WAIVING CERTAIN SUBSTANTIAL LEGAL RIGHTS WHICH I AND MY HEIRS, PERSONAL REPRESENTATIVES AND ASSIGNS, MAY HAVE AGAINST XDOT ACADEMY.
